GENIUS PDIATRIC FITNESS CENTER

 

Genius Pediatric Fitness Center provide traditional therapy services and Intensive Therapy Program (ITP).  We commit ourselves to provide professional services with a unified mission to deliver the highest quality, highest cost-effect, and highest compassionate interventions and services.  Our goal is to

" promote their maximum potential of independent mobility

in minimal time and in their natural environment ".

* Traditional Therapy Program:

We provide traditional therapy services (Early Intervention Program, CPSE related service program, and CSE related service program-RSA Program).  Treatment includes physical exercises to improve joint mobility and stability, to increase muscle strength and endurance, to facilitate balance and coordination control, and to promote functional skills. 

Therapeutic goals are focused on:

  • help children improve their mobility and stability during their activities of daily living,
  • prevent or improve the child’s permanent physical disabilities,
  • improve the child’s functional skills,
  • assess assistive or adaptive devices to reach the child’s maximum improvement (including wheelchairs, walker, crutches or orthotics).

* Intensive Therapy Program:

The Genius Physical Therapy PLLC is certified and authorized by TheraSuit® LLC to provide Intensive Therapy Program (ITP).  The Intensive Therapy Program is customed to fit the needs of each special needed child.  The Intensive Therapy Program follows the " TheraSuit Method® " which is an intensive strengthening program to treat various neurological disorders such as Cerebral Palsy, neuro-musculoskeletal or neuro-motor disorders, Grobal or Developmental Delays, and Traumatic Brain Injuries or Spinal Cord Injuries.  This method utilizes the TheraSuit® and Universal Exercise Unit combined with various treatment and handling strategies to combat the effects of de-conditioning and immobilization to promote the maximum potential of independence.

We are a fee-for-service therapy company.  We will help you to gather the information and forms for your insurance claims, but we do not provide private insurance filing.

 

*Program Options and Price -

* Free Evaluation and Consultation.
* Program A 3 hours a day/ 5days a week for 4 weeks    USD $7300
* Program B 3 Hours a day/ 5 days a week for 3 weeks USD $5500
* Program C 3 Hours a day/ 5 days a week for 2 weeks  USD $3800

 

**In order to secure your reservation at our center, we must receive a deposit in the amount of $1000.00 thirty days prior to your child’s scheduled session.  The reminder of the balance is due when your child attends.  If a deposit is not received to secure your reservation, your spot will become available to someone else.

*Payment -

We are a fee-for-service therapy company.  However, we gladly provide documentations to families for reimbursement from their insurance company.  We accept Cashier’s Checks, Money Orders, and Personal Checks.  We are currently unable to accept Credit Card payment. 

*Cancellation/Refund Policy -

This deposit is refundable if cancellation is due to medical reasons (with doctor’s note).  If you need to cancel the reservation, the money you paid will be credited to your account in our center.  Your credit will be honored for up to one full year from the date your reservation was cancelled.  You either can re-schedule or can apply your credit toward another program options within one full year from the date you cancel your reservation.

The payment amount will be refunded in full if-

  • Child does not meet the criteria to participate our Intensive Suit Therapy Program.
  • Physician does not prescribe Intensive Suit Therapy Program due to the child’s medical conditions.

* Application Process --

 

* For “Intensive Therapy Program” --

 

We provide free evaluation and consultation.  If you are interested in having your child attend an Intensive Therapy Program at our facility, or enroll in our special program options, you must follow the application process listed below:

    1. Completely fill out and return back to us the "Member Health Information Form".  It is important to fill out this form as thorough as you can.  It will provide us better understanding of your child’s strength and weakness.  This form can be mailed to you upon request, or you can get it off our web site.
    2. Upon approval for the intensive Therapy Program, you will need to submit a written report regarding recent hip x-rays (within the last 6 months), specially an A/P view of your child’s hips to rule out hip subluxation or dislocation.
    3. Once all the documentations have been received and reviewed, you will be informed by this office.  A time will be scheduled for your child to attend our program.
    4. After your child has been scheduled to attend our program, we will send you a package with information regarding hotel/accommodations, shopping, movie theater, restaurants, things to do in the area & a map to help assist you with your needs during your visit, unless you are within driving distance of our center.
    5. In order to secure your reservation at our center, we must receive a deposit in the amount of $1000.00 thirty days prior to your child’s scheduled session.  The reminder of the balance is due when your child attends.

Please follow the steps below in order to start services:
1.  Feel free to contact us at

2.  Provide us with the information of the district to which your child belongs, and the name of the district CPSE administrator.
3.  Mail, Fax, or bring page 1 and page 9 of your child’s IEP Form to our office.  Once we have received your child’s IEP page 1 and page 9, we will fill out the contract agreement, and send it back to the appropriate district CPSE administrator.  CPSE administrator will sign the contract agreement to allow all the therapy services to begin.  We will contact you to set up the regular service schedule.

* For “ CSE related service program-RSA Program” (children over 5 year-old) --

 

Please follow the steps below in order to start services:

2.  Provide us with the information of the district to which your child belongs and the name of the district RSA administrator/ coordinator.
3.  Mail/Fax or bring your child’s RSA Form to our office.  Please make sure that the child’s information and parent/guardian’s signature is completed  Once we have received your child’s RSA Form, we will fill out the contract agreement, and send it back to the appropriate district RSA administraor/ coordinator.  He/She will sign the contract agreement to allow all the therapy services to begin.  We will contact you to set up the regular service schedule. (In case that your child’s RSA Form is blank, not filled out by the Department of Education, please complete the part with your child’s information, sign at parent/guardian signature, and provide us the appropriate district RSA administaror's/ coordinator’s name and contact phone number.

UNIVERSAL PRECAUTION

 

We always maintain toys and other therapeutic devices in a clean and safe manner to prevent the spread of infectious disease, including the common cold.  We also follow the procedure of Universal Precaution

Due to the physical conditions of child with special needs, we recommended the parents not to bring your child to therapy if—

  • Any fever (99 or higher)

  • Green or yellow running nose

  • Vomiting or diarrhea due to illness

  • Any change in skin color body temperature

  • Breathing difficulty

  • Any infectious disease such as a Chicken Pox, Impetigo, Measles, Mumps, Conjunctivitis, Ringworm, Scarlet Fever, Strep Throat, and Whooping Cough.

  • Others

Please watch for any symptoms of above illness.  If your child comes down with this illness, then your child must been seen and treated by the doctor.  Please be sure to notify our center.  It the child comes to our center with any infectious disease, it is up to the clinician’s discretion to cancel the treatment session.  If there is an emergency situation, our staff will call 911 for emergency medical procedure.  Parent/Guardian will be contacted if they are not present.
